Each participant for any activity in the Stuy Dome must complete a Registration Form.  Included on the Registration Form is a standard release form that needs to be completed and given to Game Over personnel PRIOR TO any participation.  No individual will be permitted UNDER ANY CIRCUMSTANCES to participate without a completed Registration Form.  Please print out the form, complete it (if you are under 18 years of age you must have a parent or guardian complete and sign the form), and present it to a Game Over official when you enter the Stuy Dome.
